Fallout 76's main story ends with the death of the Scorchbeast Queen, which contains the Scorched plague. It is not a closed ending but a beginning, and later updates continue the story. Appalachia's future depends on the Dweller's continuing choices and on the factions they support.

outcome

The Dweller launches a nuke, defeats the Queen and becomes a hero. The plague is contained and survivors begin to rebuild. Wastelanders adds human factions and quests that extend the story beyond the Queen.

What it means

The story is about rebuilding. The Dweller arrives in an empty world and rebuilds it with recordings, allies and nukes. It asks what it takes to restore a world that has already failed twice.
